Proxy Access For Minor Patients
If you are the parent or legal guardian of a Maimonides patient who is under the age of 12, you can request proxy access to your child’s health information in the FollowMyHealth patient portal. Proxy access enables you to view some of your child’s health information in the portal and, in some instances, communicate through the portal with your child’s healthcare team. When your child turns 12, you will no longer be able to view any new information about your child in the portal.
At this time, proxy access is not available for children between the ages of 12 and 18 years old.
Proxy Access For Adult Patients
Through the FollowMyHealth patient portal, registered users can request online access to medical information for an adult in their care who has impaired decision-making abilities. Proxy access enables you to view the adult patient’s health information in the portal and, in some instances, communicate through the portal with the adult patient’s healthcare team.
