Welcome to the Pediatric Hospital Medicine Fellowship

The Pediatric Hospital Medicine (PHM) Fellowship year is structured as a mentored attending year with supervised educational activities. This fellowship provides additional training relevant to the practice of PHM in both academic and community hospital settings. This 2 -year fellowship is structured as a mentored junior attending year to provide the benefits of autonomous practice experience.

The Division of Pediatric Hospital Medicine oversees all activities in the Pediatric Inpatient Unit. The Division faculty are committed to medical education, quality, and process improvement, and family-centered care. They are very active in a wide variety of educational, quality improvement, and scholarly initiatives throughout the children’s hospital and medical center.

The Pediatric Hospital Medicine Fellowship was formed in 2013 and is the 1st program of its kind in Brooklyn and one of the first in New York City

The Pediatric Hospital Medicine Fellowship experience

The fellow is scheduled to be “on service” as the hospitalist for 1 or 2 weeks at a time throughout the year. In addition to these weeks, there are currently 2 discrete “rotations” during the year. The remainder of the time is spent in various longitudinal activities, which include learning procedural sedation, developing quality improvement and/or research projects, and participating in educational activities for residents and medical students. The Pediatric Hospital Medicine Fellowship Program

Clinical responsibilities and “on service” time

The fellow is scheduled for service for about 12 to 15 weeks and 6 to 8 weekends over the year. At the beginning of the year, the weeks occur less frequently to allow preparation time for the General Pediatrics Certifying Examination. The weeks become more frequent towards the end of the year to approach the equivalent of a full-time service attending.

Upon acceptance to the program, the fellow learns about the credentialing process for faculty attendings and can function as an independent practitioner. This is to permit the fellow to function as the “attending of record” and learn from this experience. However, at the beginning of the fellowship, a full-time faculty member is scheduled as a “back-up.” As the year progresses the mentorship gradually moves to a more indirect role.

Throughout the year, the fellow regularly debriefs with the fellowship director and other faculty about their patients and experiences to facilitate learning from reflective practice.

Procedural sedation

Throughout the year, the fellow has certain off-service weeks designated as “on-call for sedation” weeks, approximately 1 week per month. On these weeks, the fellow is in contact with the Pediatric Intensive Care Unit (PICU). The fellow assists with sedations, learning how to provide sedation for minor procedures, and will gradually be entrusted to perform them with direct oversight.

Rotations

There are 2 scheduled rotations throughout the year-long fellowship, 1 in the PICU and the other at St. Mary’s Hospital for Children:

  • PICU – The fellow spends one 4-week block in the PICU, supervised by faculty from the Division of Pediatric Critical Care. This rotation includes some overnight calls, in-house, or from home. It may include experience on the transport of critically ill children to our Children’s Hospital. The fellow performs all necessary procedures with the direct supervision of the critical care attendings
  • St. Mary’s Hospital for ChildrenSt. Mary’s is a local hospital in Queens that provides subacute rehabilitation and chronic care for children with complex healthcare needs. The fellow spends at least 3 weeks at St. Mary’s in a supervised role. The fellow spends time with each of the therapeutic services to learn about the long-term care of technology-dependent and medically complex children. Fellows also participate in intake, management, and disposition planning meetings, and learn about the role of these institutions in the healthcare system

Research

The fellow is expected to participate in quality improvement (QI) and research efforts of the division. Early in the year, the fellow will meet with our Director of Pediatric Research and the hospital medicine faculty to develop a QI project suitable to one or more Plan-Do-Study-Act (PDSA) cycles with the year, and/or create a research project that can be completed within the year. The fellow will also complete modules in the Institute of Healthcare Improvement (IHI) Open School for a foundation in QI methodologies.

The department has a monthly Research Integration in the Practice Environment (RIPE) meetings, which the fellow will attend and at which they will present their work. The fellow should also expect to work on a publication-ready manuscript and/or poster presentation for local or national research meetings, as appropriate.

Teaching experience

Teaching residents and students is a core function of daily rounds while “on service.” The fellow participates in other educational activities to hone their skills as an educator. This can include precepting a group of clerkship medical students, participating as an observer for Observed Structured Clinical Encounters (OSCEs), working with the simulation team in developing and running simulation exercises. Fellows can also participate in the interview and selection process for the pediatric residency program, to name a few.

Unlike most residency and fellowship programs, however, PHM Fellowships are NOT using the Electronic Residency Application Service (ERAS) for managing applications.
