About Our Program

The Maimonides Medical Center Home Medical Care for the Elderly Program delivers holistic, personalized, coordinated, high quality medical services to homebound frail elderly individuals in the comfort of their homes. Our nurse practitioners visit each patient at home every one-two months, or as needed, to provide a variety of health services. The treatment plans focuses on maximizing independence and functioning at home.

Who is eligible?

  • People age 65 and older
  • Homebound (limited ability to leave home)
  • Medicare recipients with chronic medical conditions such as:
    • Heart Disease
    • Stroke
    • Diabetes
    • Confusion
    • Chronic pain
    • Wounds/pressure ulcers
    • Dementia/Depression/Alzheimer's
    • Decreased mobility with risk of falls
    • Parkinson’s disease
    • Functional decline
    • Osteoporosis
    • Arthritis
    • Difficulty breathing
    • Loss of hearing/vision
    • Weight loss/Malnutrition
    • Terminal illness

What happens if I need to be hospitalized?

Our patients are admitted to Maimonides Medical Center under the care of a physician from the Division of Geriatrics. Most admissions will be to the Acute Care for Elders (ACE) unit. We will facilitate all hospital admissions and discharges.
