Please bring the following documents and information to your first appointment

For a comprehensive first visit, we recommend that you bring slides and/or doctor’s reports from previous exams, surgeries or procedures. You may need to call the hospital, doctor’s office and/or testing facility to ask for test or surgery results, including the following:

  • Pathology slides from previous biopsies or procedures
  • X-rays, CT scans, PET scans or MRIs
  • Medical summaries from your doctors
  • List of medications

In addition, please bring the following with you:

  • Insurance card
  • Photo ID card

It is also helpful to arrange for a family member or friend to accompany you on your first appointment, to help you listen and take notes.
