HISTORY OF THE BROOKLYN HEALTH HOME

The Brooklyn Health Home was established to coordinate medical and behavioral health care services. It is built on the foundation of a successful program developed at Maimonides Medical Center along with many community based organizations with funding provided by New York State. This program included Lutheran Medical Center, the Institute for Community Living, FEGS, South Beach Psychiatric Center, Visiting Nurse Service of New York and First to Care Home Care.

In 2011, additional funding allowed for an expansion of the program and organizations involved, including the Jewish Board for Family and Children Services Promoting Specialized Care and Health, Center for Urban and Community Services, and NYC Department of Health and Mental Hygiene.

In December of 2011, Maimonides was named a Medicaid Health Home in Brooklyn, now known as the Brooklyn Health Home.

Today, the Brooklyn Health Home includes over 50 provider organizations dedicated to providing high quality, coordinated care to its members.
