WHAT IS A HEALTH HOME

A Health Home is a service provided by a network of organizations working together to coordinate care. Health Homes include hospitals, medical and mental health centers, substance use treatment programs, supportive housing and more. In a Health Home, caregivers and providers communicate with one another to address your needs in a comprehensive way.
 

WHAT IS A CARE TEAM?

A Care Team is made up of a Care Manager, a Primary Care Provider, as well as a Therapist and a Psychiatrist for those who need them. We will work with the doctors you have and help you find doctors if you need them. Other members of a Care Team could include housing and social service providers, substance use treatment professionals, and other specialists.
 

WHAT IS A CARE MANAGER?

A Care Manager is your main contact in the Health Home. Your Care Manager ensures that you have access to all of the services you need to stay healthy and out of the emergency room and hospital.
