 The Affordable Care Act (ACA) has provided a number of venues to address the health of individuals and populations relevant to the mission of the Children's Hospital to improve the health and well-being of infants, children, adolescents and young adults throughout the lifespan. The law funds various demonstrations and initiatives to evaluate novel delivery system and payment models, many of which create incentives for providers to coordinate and integrate care especially for vulnerable populations. Embedded within the legislation are the terms such as population health, medical home, health home, and accountable care organizations. Read More>

Nursing
Nurse Case Managers are leaders of the MMC Case Management team, working in collaboration with physicians and other members of the healthcare team to implement a patient's plan of care and monitor the patient's length of stay in the hospital.
