How to Apply

Maimonides offers superior opportunities for growth and learning under a dedicated, superbly trained faculty. Our residents are selected from among eligible applicants on the basis of their preparedness and ability to benefit from the program to which they are appointed.

In order to pursue your candidacy, submission of the following documents are required:

1. Curriculum vitae
2. College of Pharmacy transcript
3. Three letters of recommendation
4. Letter of intent

Application Form for Admission

At least two of the letters of recommendation should come from faculty, clinical faculty or past clinical preceptors. Letters from parents, friends, or coworkers (excluding direct supervisors) will not be accepted. All applicants must be US citizens, as it is a requirement for NYS licensure. Maimonides Medical Center will not support student Visas.

Upon completion of this application process, we will contact you regarding an onsite interview at Maimonides Medical Center. Please send all required documents to:
