There are 3 measures that are known as the “Standard of Care” for the treatment of patients with Congestive Heart Failure. The goal for all medical facilities is to provide the care associated with these measures to 100% of eligible/appropriate patients.
The improvement opportunities we are currently focusing on are:
- Assure that CHF patients receive education and discharge instructions regarding diet, activity level, medications, follow up appointments, daily weight measurements, and signs and symptoms.
CORE MEASURES: Heart Failure
Appropriate instructions for patients regarding activity level, diet, medications, follow-up appointments, daily weight measurement, and monitoring of symptoms is important in managing heart failure.
Left ventricular Function Assessment
An evaluation of the LVS function checks how the left chamber of the heart is pumping.
ACE-I or ARBs for LVSD
ACE inhibitors and ARBs are medicines used to treat heart attacks, heart failure, or a decreased function of the heart.
