Insurance approval can be a time-consuming process, but it is necessary. Our office staff will work with you to ensure your surgery request includes all the appropriate documentation for consideration. The following list is the minimum amount of information required for most insurance carriers to process your request for surgery:
- Blood levels
- Normal thyroid function tests
- Psychological evaluation
- Nutrition evaluation
- Surgeon - Letter of Necessity (written after the surgical consultation)
