The Congestive Heart Failure Team

The Maimonides CHF team consists of a physician director, nurse practitioners (NPs) and registered nurses. Our team assesses your cardiac needs and individualizes a plan of care based on those needs. Maimonides offers the latest in CHF treatment modalities and strategies, including, but not limited to, coronary artery revascularization procedures, defibrillator implantation (AICD), biventricular pacemaker implantation and current effective use of medication. Our Joint Commission-accredited Left Ventricular Assist Device (LVAD) Program is also available for patients who may be suffering from advanced heart failure and who are not eligible for a heart transplant.

The CHF team works closely with referring physicians and pays special attention to every detail to make sure your experience is comfortable.
